Location: Wayne State University, Eugene Applebaum College of Pharmacy and Health Sciences – Occupational Therapy Department. All occupational therapy services will be delivered in the Rosa Parks primary care clinic in Detroit, MI or in the patient's home.

Job Purpose: Provides traditional occupational therapy treatments to pre-frail older adults as part of the research Frailty Prevention Program (FPP).

Essential Functions:
-Evaluate patients' conditions and needs. Develop and implement treatment plans that aid in patients leading a more independent and active life; apply behavior change strategies and health behavior education.
-Educate patients, patients' family and/or caregivers on treatment plans.
-Document services delivered during the project using ICD 10 codes.
-Participate in required training and educational activities including but not limited to: dietary and physical activity recommendations for adults; evidence-based strategies to promote habit development; and the principles of motivational interviewing.
-Participate in staff study meetings.
-Complete required research documentation: treatment session length, frequency, and content delivered.

Qualifications:
Education:
Bachelor's degree in Occupational Therapy. Licensed as an Occupational therapist in the state of Michigan.

Experience:
Minimum three years of full time patient care experience. At least two years of experience working with older adults.

Knowledge, Skills, and Abilities:
Project Management: Ability to independently manage a variety of projects while ensuring appropriate tracking, quality control, follow-up and multiple deadlines are met. Strong time management.

Communication Skills: Ability to communicate clearly, concisely and professionally, both orally and in writing. Strong presentation and training skills.

Planning and Organizational Skills: Ability to independently manage a patient caseload, including scheduling and follow up with patients as well as documenting results of patient evaluations, progress notes and discharge documentation, and communication with the primary physician and study staff. Demonstrated capacity for multi-tasking, flexibility in face of changing priorities, and the ability to work independently as well as part of a team. Prioritize and plan work activities; adapt to changing conditions and develop strategies to achieve organizational goals.

Computer Skills: Proficient in Microsoft Word.

Must have own transportation with a valid driver's license and proof of automobile insurance.
