Recurring Visits Form - We will schedule pet therapy at your facility on a regular schedule.

Please note:

  • Location: within our service area to ensure adequate coverage
  • Length of visits: 1-1.5 hours long
  • Number of teams: (a team is a certified owner and pet) 
    • Usually consists of one or two teams
  • Notice: a minimum of 2 weeks before the first visit date
  • Fees: (cover the cost of programming and insurance). CCPT staff will discuss the fees with you.
